Participant Ticket & eTicket Worksheet
Roll mouse over INDEX to the left to select your form
Select the "Ticket Worksheet" to access a fill-in form that you can fill-in on line, print, and give to your Troop Guide. You may also print the form use pen or pencil to fill it in, and give it to your Troop Guide.
The page that opens "eTicket Worksheet" is an on line fill-in form, that is sent to your Troop Guide automatically. Enter the information on the appropriate lines or select from the "pull-down" menu. To use this form you will need the login information provided by your Troop Guide.
You must have a JavaScript runtime module installed (Get it here) to use these forms.
Once you have completed the form use the boxes at the bottom of page to “Submit” or “Reset”. If you select “Submit” then your form will be available to the staff by email (The form will be automatically addressed). If you select “Reset” then your form will be cleared.
